Why Midfoot Sprains Are Easy to Miss
The midfoot is a complex set of small bones held together by strong ligaments. When those ligaments are injured, the bones can shift in ways that don't always show up on standard, non-weight-bearing X-rays. The most serious midfoot injury, called a Lisfranc injury, can leave the foot looking essentially normal on initial imaging while the underlying joint stability is compromised.
The result: patients often leave urgent care with a "looks fine, take it easy" report, only to find weeks or months later that the pain isn't going away. By the time the injury is properly diagnosed, the treatment options are narrower and the recovery is harder.
How We Diagnose Midfoot Sprains
Thorough Physical Exam
We examine the midfoot for the specific signs of midfoot injury: tenderness along the tarsometatarsal joints, swelling along the top of the foot, bruising along the arch on the bottom, and pain with specific stress tests.
Weight-Bearing and Stress Imaging
We use weight-bearing X-rays, stress films, and angled radiographs that can reveal joint instability standard X-rays miss. These views can show widening between bones that only appears under load.
Advanced Imaging When Needed
For cases where the diagnosis remains unclear, we coordinate advanced imaging (CT or MRI) promptly. CT scans show subtle joint disruption clearly. MRI shows soft tissue injury directly.
How We Treat Midfoot Sprains
Stable Sprains: Boot Immobilization
For stable midfoot sprains, treatment is typically boot immobilization with protected weight-bearing for several weeks, followed by gradual transition back to normal shoes and activity. We monitor for any signs of instability throughout recovery.
Unstable Sprains: Surgical Fixation
For unstable injuries, including most Lisfranc injuries, surgical fixation is usually required to restore the proper alignment of the midfoot joints. Without surgery, an unstable midfoot tends to develop early arthritis and chronic pain. We perform these procedures at Northern Rocky Surgery Center.

Midfoot surgery recovery is gradual. Our in-house physical therapy team coordinates directly with your surgeon throughout the protected weight-bearing phase, the return-to-loading phase, and the return-to-activity phase.
